Lakewood Hospital SeniorCare Services is dedicated to meeting the growing needs of older adults. The team provides a variety of services at multiple levels of care that benefit the healthy aging and the specialized care needs of frail seniors.
SeniorCare offers an extensive range of programs to enrich lives and provide guidance for families and caregivers. The programs and services are available within the hospital and at our Community Health Center located at 1450 Belle Avenue (across from the hospital's main entrance) and at additional locations throughout the Westshore area.

The SeniorCare Assessment Center is a cornerstone of our program. The expert staff provides comprehensive outpatient services for older adults experiencing a change in their physical, psychological or social well-being. The team works with older adults, their family members and the patient’s primary physician to develop and individualized care plan. The Cleveland Clinic Driver Rehabilitation Program at Lakewood Hospital is for adults who have experienced any physical, neurological, visual or cognitive change in function. This driving assessment is conducted by an occupational therapist trained as a Certified Driver Rehabilitation Specialist. The program provides a clinical and on-the-road assessment of driving skills, with recommendations to the client, family and physician. For more information, please call 216.521.2228.
