Who we are:
We are a team of caring professionals who address and coordinate all aspects of the older adult’s care. Our team consists of geriatricians, a board-certified medical doctor with specialized training in preventing and treating health problems of the older adult; geriatric psychiatrists, a psychiatrist trained to deal specifically with the mental health concerns of an older adult; a geriatric clinical nurse specialist; registered nurse; and social worker all dedicated to working with older adults.
Our goal is to complete a comprehensive assessment that addresses the specialized care needs of both the older adult and their caregivers. We provide support, education and recommendations for future planning by working with the patient's primary care physician to develop a plan so the patient and family can successfully manage their care.

Our specialized team can offer guidance in managing many common problems older adults face such as:
· Decline in ability to carry out usual activities
· Memory loss
· Depression and anxiety
· Medication problems
· Falls or difficulty walking
· Concerns regarding living arrangements
· Caregiver stress and concerns

· Driving safety
Additional services available:
Geriatric Psychiatry Consultation:
If needed, a thorough consultation by a geriatric psychiatrist will offer expert care and treatment for older adults who are experiencing behavioral changes.
The Cleveland Clinic Driving Evaluation & Rehabilitation Program:
A clinical evaluation and on-the-road assessment of driving skills is conducted by an occupational therapist.
