Our outpatient Anticoagulation Clinic provides a special place for patients who are taking blood thinning medication, such as Coumadin® to receive one-on-one care. The clinic is designed to help patients manage their medication regimen as safely and accurately as possible.
Patients who come to the Anticoagulation Clinic will receive comprehensive care by a specialized team of clinicians including highly trained nurses certified in anticoagulation therapy, pharmacists and lab technicians.
The clinicians will:
- Provide assessments
- Review medical history and diet
- Administer a simple finger stick blood test
- Monitor lab results
- Review current prescription medications, over-the counter medications and supplements to check for possible drug interactions
- Adjust blood thinning medication dosages as necessary
- Communicate results with the patient and their doctor
- Provide individualized and/or group education
- Review safety measures
