The initiative may be able to help you with your diabetes education and/or diabetes medication and supply needs. There are funds available for prescription diabetes medication; supplies; and scholarships for diabetes education classes for Ohio residents falling at or below 400% of the Federal Poverty Income Guidelines.
Educational Scholarships
The key to successfully managing diabetes is learning about what it is and what you need to do to control your diabetes. Classes are taught by a certified diabetes nurse educator and licensed dietitian.
Diabetes Medication and Supplies
Vouchers available for coverage for limited time. During this time, individuals will be assisted with completing paperwork for medication assistance programs offered by various pharmaceutical companies.
