Residents of our facility, their family and friends can rest assured knowing that our specially trained, dedicated professionals are providing quality care 24 hours a day. Take comfort in knowing that the comprehensive services, medical expertise, testing capability and advanced technology of Lakewood Hospital are nearby should they be needed or if an emergency occurs.
While the Skilled Nursing Facility is located in the hospital, it operates differently. Our residents are well on their way to recovery and no longer need acute hospital care. The Skilled Nursing Facility helps residents make the transition from the hospital to their normal living environment, so the facility’s atmosphere is related and comfortable. Most of our residents can continue under the care of their personal physicians. However, if that is not possible, our medical director will take over to ensure that each resident receives quality care and treatment.
Plan of Care Team Assures Comprehensive Care
Our Plan of Care Team, which includes a licensed nurse, dietitian, social workers, activities director, and rehabilitative therapist, helps ensure that residents have the maximum support and expertise to enhance their independence and self-reliance.
Family Friendly Atmosphere
Family members are encouraged to participate in the resident’s progress and to take an active role in the rehabilitative process. Family and friends find our 24-hour visiting arrangement convenient for their schedules, allowing them the opportunity to provide support to assist in the rehabilitation and recovery of their loved ones.
Guidance During Admission and Discharge
Our social workers provide help and counseling to assist residents, family and friends with the special issues that may be associated with admission to a skilled nursing facility. The social workers also will assist with all phases of discharge planning so residents return to normal environment as quickly and easily as possible.
Therapy and Special Services
We realize our residents have many varied needs, and we remain prepared to meet them. Physical, occupational and speech therapy are available as prescribed by a resident’s physician and special medical services such as podiatry, optometry, audiology and dentistry can be conveniently arranged.
