Leaving the Hospital / Post-Acute Services

Discharge Instructions


Plans for discharging patients will begin as soon as possible during the patient’s hospital stay. Once the discharge plan has been set, every effort will be made to discharge the patients within 2 hours of the discharge order. Please discuss discharge questions with your physician, nurse or social worker. You may also call the Patient Representative at ext. 7049 with any questions.

Your physician and nurse will give you written instructions on your care after discharge from the hospital. Please be sure to ask any questions about medications, diet, activity, or other health concerns at this time. Please be sure you have checked your room, closet and the table over your bed for any belongings. If transportation home is necessary, please inform your nurse who will coordinate this need for you.

Discharge Lounge


In an effort to streamline the patient flow process you may be transferred to our Discharge Lounge once your physician has written discharge orders. The lounge is located on the first floor of the hospital, near the main entrance. Your transportation can pick you up at the Belle Avenue Main Entrance to Lakewood Hospital.

The lounge is equipped with comfortable recliners, telephones, refreshments, restroom, television, and patient educational information. For your convenience, the Discharge Lounge is staffed with a team of healthcare professionals who can assist you with scheduling follow-up appointments, discussing any home-going instructions and instruction on your patient education needs.

Home Care


Home is where we find the most comfort – after a hospital stay, while rehabilitating, or simply going about day-to-day activities. At home, you may need a helping hand to take care of you or a loved one. For information on the Home Care options available, ask your nurse or case manager.
